Instructions for Family Housing Request

Form 1c -- Family Housing Request

This form was designed for assigning and billing phones to family housing.  The following information is necessary for the work to be performed:

1. Building or Address where the work is to be completed.
2. Applicable rental and/or installation index/activity codes or student ID.
3. Reasonable requested completion date given the type and quantity of work requested. If none listed, it will be assigned next available date.
4. Name of regular or alternate occupant who is submitting this request.
5. The phone number(s) of the above individual(s). This may include extensions, pagers, off-campus numbers, etc.
6. Special access information, if necessary.
