Form 15 -- Meet Me Service Request

University of Oregon generated Conference calls. 

• Department Contact Name: Name of regular or alternate coordinator who is submitting this request. Give two names if the regular coordinator may be out of the office during the work schedule.
• Contact Number: The phone number(s) of the above individual(s). This may include extensions, pagers, off-campus numbers, etc.
• User Name: Individual who will be "chairing" the call.
• Name of the Department.
• Index: Index to be charged for equipment and recurring charges.
• Activity Code: as required by your accountant.
• Activation Date:  the date the conference call is needed
• Sundry details as needed
