Form 14 -- Bridging to Cellular Service Request
Bridging to cellular is also known as XMobile or EC500 service. This service is a link between The University of Oregon phone switch and a given user's cellular phone, such that the user may receive office phone calls on his/her cellular phone.
- Department Contact Name: Name of regular or alternate coordinator who is submitting this request. Give two names if the regular coordinator may be out of the office during the work schedule.
- Contact Number: The phone number(s) of the above individual(s). This may include extensions, pagers, off-campus numbers, etc.
- User Name: Individual who will be using the Cellular Phone or Pager ordered.
- Name of the Department.
- Index: Index to be charged for equipment and recurring charges.
- Activity Code: as required by your accountant.
- UO phone extension the calls will come from.
- Cellular phone number the calls will go to**.
**Please note if user does not have cellular service at this time, you will need to fill out the F12 form for cellular service in addition to the F14.
