Instructions for Telecommunications Order Forms
Form 8 -- Special Order for Accessories
This form is used to order any non-standard accessories/devices the Departmental User requires. Items include Hearing Amplified Handsets, Headsets, Speaker Phones, handset and mounting cords in different lengths, etc. Please contact the Telcom Help Desk at 346-NETS for more information and price quotes.
It is mandatory to fill out the Coordinator name, Department name and the billing Index/Activity codes.
