Instructions for Telecommunications Order Forms

Form 2b -- Order Form, Move Service

This form is a summary of features and equipment for existing extensions that will be moved. Please fill out all data fields except for 1 and 8. Please note that each data field type is identified by a number located above the field name.

Please be sure to indicate the rental/installation index/activity code numbers that this order will be charged to. Do not use this form to change account billing information, Form 9 is the appropriate document.

  1. Prepare a separate telephone packet for each index/activity code.
  2. Grant accounts must also include the activity code.
  3. There is no charge for name changes, Voice Mail changes, or deletions; however we still need the account information for a cross reference.

Field 1 -- Action Code, has already been filled in for you.

Field 2 -- Extension Number

The telephone number of the service that is moving. This could include campus extensions, payphone numbers, FTS lines or any other "outside" line which accesses the UO cabling to provide service to the user.

Field 3 -- Set Type

The type of set or jack (in the case of data or fax) that is to be moved. See Set and Jack Definitions for a list.

NOTE: If you want to change/move a single line set to a digital or a digital to a single line set, you will need to use the Form 2a and Delete the Existing line/set, then (one the same form) Add in the line with the new set designation and location.

Field 4 -- User Name

The name associated with this service.

Field 5 -- Building or Address

The name of the current building or address ("from") and the name of the building or address where the service is moving to ("to").

Field 6 -- Room or Suite Number

The old room or suite number ("from") and the new room or suite number ("to") where the service is moving to.

Field 7 -- Jack #

Many of our buildings across campus have been re-wired and new jacks have been added. These new jacks have been numbered, and have been coded to give the technicians the actual physical "address" of the jack which can trace the wiring from the jack all the way back to the Oregon Hall Switch Room. If you building has been re-jacked, these numbers must be included on this order. Please use the full, eight-character jack number. See sample jack.

If your building has not been re-wired with the new jacks, it is very important to include a map with your order for new location, but maps of the old location are not necessary.

If you wish to move service to an non-jacked location and need to have a new jack added to the building, you must send us a map of the room(s) involved and the exact location(s) of the jack(s) you require.

Field 8 -- Port Number, LEAVE BLANK.
